Appointment Checklist
1) Bring any prescription medications you are currently taking with you to your appointment.
2) Write down any questions or concerns you might have so you don't forget to bring them up during your exam.
3) Please turn off your cell phone when you enter the exam room.
4) Feel free to bring a family member or friend with you into the exam room to help you communicate your concerns or questions.
5) If you are unable to keep an appointment, please call and cancel it.
