Admission Process

You and your family should feel free to discuss hospice care at any time with your physician, other health care professionals, clergy or friends.

If you contact us directly, we will contact your physician to make him or her aware of your request for hospice care. If you have no primary physician, our medical director is available to help you.

We will visit you and your family at home, in the hospital, or wherever you are to obtain your authorization for hospice care. You will be asked to sign consent and insurance forms, including a Hospice Election Form that says you understand hospice care is palliative (aimed at pain relief and symptom control), rather than curative.

By law, the decision to enter hospice belongs to you. We also will discuss your advance health care directive, in which you give instructions for your health care when you are unable to make a communicate those decisions for yourself.
