The Heart of America Living Center is here for you when it matters most. Whether you have an injury or illness and need a few weeks to recuperate, or you need a safe and caring place for transitional care, we can meet your needs for skilled nursing care.
Nationally recognized as a quality care provider, our 16-bed facility is located on the North Side of the Heart of America Medical Center.

Heart of America Living Center is licensed to provide:

- Skilled nursing care
- Medical care
- Restorative Care
As a Heart of America facility, we provide worry-free continuity of care and can quickly adapt to your changing needs.
Heart of America Social Services coordinates smooth transfers to our facility from Haaland Estates and Heart of America Medical Center. If a medical emergency should arise, the hospital is just an elevator ride away.
We are happy to accept patients from other facilities when we can meet their needs.
Restorative and Rehabilitation Services
To help you achieve your highest level of independence, the Living Center nursing staff works with physical and occupational therapists to develop personalized restorative programs designed to help you stay mentally sharp and physically fit.
Physical Therapy

- Lower extremity strengthening exercises
- Transfer training
- Advanced wound therapy
Occupational Therapy
- Upper extremity strengthening, range of motion and coordination activities
- Training in activities of daily living
- Splinting

- Wheelchair and mealtime positioning
- Lymphedema therapy
- Parkinson’s Disease treatment
Living Center patients also have access to the Heart of America Wellness Center.
Care Center Frequently Asked Questions
What is the difference between transitional care and basic care?
Residents of basic care facilities like the Haaland Estates Basic Care, can live fairly independently and manage their activities of daily living with limited assistance, while interacting appropriately with other residents. Heart of America Living Center is the tranistion between Basic Care and a Long Term Center.
What if my care needs change?
Your plan of care will be reviewed at a quarterly care conference. You and/or your family are invited to attend care conferences every six months, or more often if your needs change significantly. Additional care conferences may be scheduled at any time by contacting Heart of America Social Services.
Can I keep my same doctor?
No. The care during transitional care is done by Dr. Hoff, MD.
Do I give up my rights when I enter Heart of America Care Center?
Patients' rights remain intact and are guaranteed by state and federal law.
