Monday through Friday, 8:00 am to 5:00 pm

Before Your Appointment:

Plan to arrive 15 minutes before your scheduled appointment.
Bring all prescription medications and supplements you are currently taking, or a list of all of them, with you.
Write down any questions or concerns you might have so you don't forget to bring them up during your exam.
Find out if you are due for any general screenings or vaccines.
Talk to your provider about all health and life changes since your last visit.
Feel free to bring a family member or friend with you into the exam room to help you communicate your concerns or questions.

If you are unable to keep an appointment or if you may be more than 15 minutes late, please call the office as soon as possible to cancel or reschedule.
