Co-payment: This is a fixed amount paid by you to your clinic/hospital at the time of service. Your co-payment amount is usually listed on your insurance card. This amount is never covered by your insurance company and is not applied to your out-of-pocket co-insurance or deductible amounts. Your insurance company expects that this amount will be collected by the provider.
Deductible: This is a specific amount you must pay, after your co-payment, before your health insurance will pay any of the amount submitted. There is normally a deductible for each family member insured under your policy, with a maximum family deductible each year.
Co-Insurance: This aspect of your insurance will take effect after the co-payment has been paid and the deductible has been met. Your insurance will split the amount due, usually 80-20, with you. In an 80-20 split, your insurance pays 80% of the bill and you pay 20%. There is usually an out-of-pocket limit for co-insurance. Once you have reached your out-of-pocket maximum, your insurance will cover 100% of the costs thereafter until your new place year begins.
Self-Pay: If you do not carry insurance, you will be responsible for full payment of your bill. Heart of America Medical Center does offer a Prompt Pay discount if paid within 30 days of the first bill date. This does not include long-term care services.
Financial Assistance (FAP) and Community Care Programs: If you meet financial requirements, you may be eligible for a discount on your financial costs. Community Care is based on income and applied on a sliding scale. FAP is based on income only.
Heart of America Medical Center provides care to all regardless of ability to pay. No one will be denied services based on their ability to pay. Discounts are available based on income. Patients who meet financial guidelines may be eligible for free care through our Community Care Program.
To apply, please contact one of our patient account representatives at (701) 776-5261. A downloadable PDF of the income guidelines is below.

Your Financial Responsibility
Our facility will file insurance claims on your behalf. This service does not release you from any responsibility for the charges on your account. Your insurance contract is between you and your insurance company.
Our facility will allow your insurance company reasonable time to process your claims and remit payment. Please recognize that we have no authority or responsibility with your insurance carrier.
Many insurers limit payments to the “usual, customary, and reasonable payment.” We do not accept payment limitations from insurance companies with whom we do not participate or have contractual arrangements.
Co-payment is preferred at the time of your visit. Your co-payment amount, along with other necessary insurance/third-party payer information, is usually found on your insurance card, so please bring your insurance card with you. Payments may be made by cash, credit card, or check.
If you do not have insurance coverage or have balances due after your insurance has made its payment, you will be responsible to make appropriate arrangements with our Patient Account Representatives.
As part of our mission, Heart of America Medical Center provides care to all regardless of ability to pay. Financial assistance may be available on a case-by-case basis, after the submission of all requested information to the Business Office. Please contact one of our Patient Account Representatives in our Business Office to discuss your payment options at (701) 776-5261.
As required by CMS a list of our charges may be found within the MyChart Patient Portal. Don't have a login? No need to worry, this information can be accessed through Guest Estimates.
Step-by-step instructions if you do not have a login:
Click here to access the MyChart Patient Portal
Now you can type in keywords to find the service you are looking for. For example, CBC, or you can click one of the categories to locate it. If you have insurance coverage you can select your insurance provider or click the Skip this step if you do not have insurance. The estimate of your charges will appear.
Please note that the charge is not always what you need to pay, but may be an estimate. Charges may be different because of third-party agreements, like health plan agreements, agreements with government payers, and our charity care program. We will assist you as best we can to help you determine what you would pay for services done at Heart of America Medical Center.
